Documents you need to send to us:

  • Original tickets to be refunded
  • Copy of your invoice, or invoice number on your letterhead
  • Original hospital certificate, or copy of death certificate if applicable
  • Your comments or special requests

Once we receive all these documents, we will fax you a receipt number relating to your case.
Please reffer to this receipt number for further inquieries on the status of your refund.

Please note that all refunds will be subject to penalties.
